Start with Québec Health Booklet
Québec Health Booklet is the official online service for eligible residents to view selected health information. The official page lists medications obtained from pharmacies, laboratory results, medical imaging reports, selected RAMQ service and reimbursement information, and a record of health workers who consulted the information. It also connects to selected appointment and family-doctor services.
Check eligibility before creating access
The official service requires a health insurance card. To access it, the official page says to authenticate using the Government Authentication Service or create an account. A person must generally be at least 14 years old and not be under guardianship, trusteeship or in a situation where another person acts as their representative. Use the official page to confirm the current rules and instructions for trusted-person access.
Know what the service does not promise
Not every clinical note, document or result necessarily appears in the online service. If information seems missing or incorrect, use the support, access or correction route identified by the official operator or the organization that holds the record.
Use the Québec Health Record route when needed
For information in Québec Health Record databases, the official rights page describes rights to access and request correction. It also explains the formal access steps for people who cannot use Québec Health Booklet. This is not presented as the complete access route for every medical record held by every clinic or institution. Do not submit personal health information to Teché or the Navigator.
